The Health Coach position is responsible to provide health coaching, management and education of members with chronic care conditions. This position also assists in efforts to improve HEDIS rates, improve health outcomes, support the CMS Chronic Condition Improvement Program, NCQA accreditation and the CMS Stars initiatives.
Provide telephonic health coaching and management to members with chronic conditions, including program engagement calls.
Collaborate with the Program Coordinator and Engagement Specialist for timely contact of identified members.
Perform outreach calls to members for assessment of disease state and program enrollment.
Assist in the development of Disease Management (DM) educational materials used for health coaching purposes.
Implement strategies to reach and maintain DM program goals, improve HEDIS rates, support the CMS Chronic Condition Improvement Program, NCQA accreditation and CMS Star Ratings.
Enter member information in the Care Enhance Clinical Management (CCMS) clinical documentation system. Documentation is accurate, thorough and timely.
Assist in the development and ongoing support of measurement systems to evaluate DM program outcomes and processes. Assist the Program Coordinator with data for the monthly report as requested.
Implement health coaching for members of various socioeconomic and cultural groups.
Serve as a resource to staff, providers and members enrolled in UCare administered DM programs. Provide ongoing education and information about DM programs as requested.
Other projects and duties as assigned.
Registered Nurse (BSN preferred) with current unrestricted RN license in Minnesota and Wisconsin required
This position requires two years experience in disease management or chronic condition care, along with experience working with individuals who have chronic conditions that foster self-management.
Our ideal candidate will have managed care experience with a focus on health coaching individuals with co-morbid conditions. Experience working with multi-cultural and variable socioeconomic populations highly desired.
Hmong/Spanish speakng preferred
Work Schedule - Monday - Friday 8:30am - 5:00pm
